Sierra Nevada Home Health Care (SNHC) offers a wide range of home care services. The Hospital's Home Care Coordinator assists patients and their families to arrange home care. They possess in-depth knowledge of surgical procedures and oncology care issues, offered within a multi-disciplinary coordinated care model. Services include nursing, physical, occupational and speech therapy, medical social services and home health aides for personal care on a part-time, intermittent basis. Nutritional services and home medical equipment are also available by arrangement.
The agency provides specialized nursing services including home IV care, pain management and Enterostomal Therapy care (ET). The ET nurses provide personalized attention, instruction and counseling for persons who have, or may have, a stoma as a result of surgery. They also evaluate, recommend and/or provide care to patients with incontinence or complex wound and skin care problems, as well as those needing specialized beds for comfort and healing measures.
Continuous home care from RNs, LVNs and nursing assistants is available from 2-hour to 24-hour timeframes and is provided by Home Care's Supplemental Services Department.
Lifeline
Sierra Nevada Home Care also offers Lifeline, a personal emergency response program especially helpful for those home alone.
